The Ministry of Health is creating a database of Ontarians with Type 1 and Type 2 diabetes. This is part of an initiative to improve screening of diabetes in 3 areas: testing of glycosylated hemoglobin, testing of LDL cholesterol, and eye examinations. They have requested that all primary care physicians provide a list of their diabetic patients by June 11, 2010.
The Ministry will keep a registry of the dates patients have had the 3 screening tests, and will send that information to the family physicians. That way, each doctor will know which of their diabetic patients are due for testing and when.
To opt out this program please call the Ministry of Health at Service Ontario,
1-800-291-1405 after June 11, 2010; or 1-800-387-5559 on or before June 11, 2010.
